Our Program Purpose:
The grant provides free or low-cost healthcare and supportive services to people who are recently released from incarceration or preparing to return to the community. The goal is to help individuals successfully transition back into the community while addressing their healthcare and social needs.
Who It Serves
- Individuals recently released from incarceration
- Individuals preparing for release/reentry
- People who previously enrolled in the program but have not recently received services
- Individuals who may be uninsured or have difficulty paying for care
Services Provided
The program connects participants with:
Primary/medical care and checkups
Medication refills and affordable prescriptions
Mental health counseling/support
Dental care
Case management and peer navigation
Transportation assistance
Housing and community resources
Medicaid enrollment assistance
Substance use treatment
Referrals to additional community services
Key Benefit
The program is designed to remove barriers to healthcare after release. Participants can receive assistance even if they do not have insurance or cannot afford care, with the flyers emphasizing that everyone is welcome and patients will not be turned away simply because they cannot pay.

Southside Medical Center is serving as the healthcare and community-support provider, offering participants a point of connection for medical care, peer navigation, resources, transportation assistance, and help reconnecting with healthcare.
